MS Word document Index links don't convert - using Adobe PDF Create online

i'm converting word xp office doc pdf using 'create adobe pdf online facility. when create pdf version, notice entire index inactive - ie, pdf document's index links fail convert index can't clicked on @ all.

i thought may have been optimization level. used 'optimize e-book first , got problem, next tried 'optimize web' still doesn't work.
can tell me i'm doing wrong?
